Mission Statement: ElderHelp’s mission is to provide personalized services and information for San Diego’s growing population of low and moderate income seniors to help them remain independent and live with dignity in their own homes.

Advancing Health and Wellness: Our mission is to provide personalized services and information that help seniors remain independent and live with dignity in their own homes. The Concierge Club was formed as a resource for the growing number of low to medium income seniors in San Diego needing assistance with day to day living. The Concierge Club has grown to service 243 members supported by 555 volunteers. As the aging population grows, there is more demand for our services resulting in a wait list for some seniors as well as the need for additional funding. Membership provides opportunities for seniors to improve their physical and mental health through socialization, recreational activities, access to healthcare and in-home health education. Services include health and financial advocacy, home care provided by volunteer caregivers, case management, referrals, grocery shopping, escorted transportation, home safety inspections, minor home repairs, light housekeeping, companionship, and pet care.

Description: The Concierge Club is the evolution of ElderHelp’s core case management, community partnership and volunteer caregiver programs. Membership provides opportunities for seniors to improve their physical and mental health through socialization, recreational activities, coordination and access to healthcare, in-home health education and prevention activities. The organization has mobilized and expanded a paraprofessional volunteer workforce that is augmenting the need for paid caregivers and assisting overburdened families. In 2012, ElderHelp successfully provided services to 4,418 seniors whose goal was to remain living in their own homes. The Concierge Club has grown to service 247 members supported by 301 direct service volunteers who provide service in the home as well as transportation. Our services include health advocacy, home care provided by paraprofessional volunteer caregivers, high level case management, trusted referrals, grocery shopping, escorted transportation, home safety inspections and modifications, minor home repairs, light housekeeping, companionship, financial advocacy, and pet care. The CDC reports that the demands associated with long-term care might pose the greatest challenge for both personal/family resources and public resources. ElderHelp is one of the only non-profit agencies in California that is solely focused on providing affordable services and programs to seniors within their own homes. Without our services, these seniors face tremendous challenges including a high and rising cost of living, fragmented services and unreliable transportation. As the aging population continues to grow, there is more demand for our services which has led to other villages across the nation looking to us as a model for best practice when addressing the issues of long term care.

Advancing Health and Wellness: The Concierge Club is a cost effective monthly membership program for low income Medicare and MediCal beneficiaries that provides home based social supports enabling seniors to access better quality healthcare. A 2010 poll from the Robert Wood Johnson Foundation highlights that four out of five physicians believe that unmet social needs are leading to poor health. The Concierge Club bridges the gap between social support and medical care. Concierge Club benefits include case management, health advocacy, and home care provided by paraprofessional volunteer caregivers. Seniors with incomes below $13,000 pay $0 while those over $13,000 pay a membership based on their income.
